We carefully consider your enquiry to ensure we can meet the needs of the young person and take due care regarding the matching process to safeguard all young people currently placed with us.
How to make a referral
We select the most appropriate setting based on: the needs, age and other information you provide us with. In response to an enquiry we create an initial placement plan and risk assessments, which are an overview of how we will care, safeguard and work with the young person. Our considered process facilitates pre-placement visits for all involved.
Call the referrals team on 0808 196 8219 and we will ensure your enquiry is handled efficiently. If your call is out of hours, or the line is busy, you will be automatically transferred to the duty mobile.
We are part of the Polaris Community. To refer via email please contact us at residentialenquiries@polariscommunity.co.uk. Please attach the latest: CLA Reports, Health Reviews and chronology, Assessments, EHCP or any reports from other professionals involved in supporting the child. A chronology is really important as it informs the initial approach we undertake and which core elements of the therapeutic model we focus on.
Schools
Placements are agreed with the Local Authority SEND teams. Should you wish to discuss a school place, please contact your Local Authority SEND team.
